New Patients

Our practice list is currently open. We are happy to accept patients who live within the Practice Area.

The easiest way to join the practice as a new patient is by completing an Online Registration Form for each member of your family wishing to register https://gp-registration.nhs.uk/C81089

If you have completed the paper form, please bring to Reception or email to ddicb.stubleymedicalcentre@nhs.net

Whilst not legally required it is our practice policy to confirm the identity of adults registering with us by checking photo ID and proof of address, please also provide this when registering, if this is possible.

As a new patient, you’ll be invited to attend a Health Check appointment with the nurse – this is important.
